Postpartum Alopecia


Childbirth can be a stressful time and for some women it can result in a temporary hair loss condition called postpartum alopecia (also known as postnatal alopecia). The condition usually starts around two to three months after childbirth, gradually worsening for up to three months then recovering over the next three months until hair starts to re-grow. Postpartum alopecia is characterized as a general thinning (also known as diffuse thinning) over the whole scalp. Here's a great exercise to tone your shoulders: Stand with your feet shoulder width apart, knees slightly bent, pelvis tucked under, and chest and head lifted. Place one weight in each hand and rest them with your arms straight on your upper thighs. Keeping your arms straight (only a slight bend in the elbow), lift your arms up so your arms are parallel with the ground and your wrists are even with your shoulders. For best results use 2-3 lb. weights and work up to 2-3 sets of 15-20 reps.

Extend your right leg straight back behind you- your right toe is pointed and touching the ground. Inhale then exhale lift your right leg up straight as high as you can without leaning forward. As you lift your leg tighten your buttocks. Work up to 3 sets on each side-10 slow lifts, 10 pulses, 10 slow lifts.

When you're pregnant there may be times when you ate a couple of hours ago and you're hungry again. Listen to your body and EAT! Just chose healthy snack foods like a few handfuls of Kashi Go Lean Crunch cereal, or carrots, or grapes with cheese. When you listen to your body, you're nourishing yourself and your baby and that means you'll have more energy, better mood, and you'll lose the baby weight quicker.

7. Pregnancy Exercise- Tone Your Thighs

Here's a simple pregnancy exercise you can do to tone your quads (top of thigh) and inner thighs. I call the exercise "Squat and Sweep". How to do the move: Hold onto the back of a chair with your feet approximately 3-4 feet apart and your toes slightly turned forward. Bend your knees into a pilee squat (make sure your knees never go over your toe) and keep your back straight, head lifted. As you press back to stand bring your weight towards your left leg and slide your right foot on the ground (i.e. "sweep") so it crosses over your left supporting leg. Next sweep your right foot back into a pilee squat. As you press to stand, sweep your left foot across your right leg. Repeat 10-20 reps each side.

When you're pregnant it's really important that you focus on nourishing yourself and your baby. When you listen to your body and eat healthy food there's no need to worry about gaining excess weight that's hard to lose. Always bring healthy snacks with you to avoid getting over-hungry. My favorite on the go prego snacks include: nuts, raisins, (walnuts and raisins together are yummy!), granola bar, and string cheese.

What are Section 125 Plans?

One type of group insurance benefits that may be offered by an employer are Section 125 Plans. These Section 125 Plans are also known as cafeteria plans, flexible benefit plans or mini-flex plans. The Internal Revenue Code Section 125 outlining these plans first appeared in the tax code in 1978, but didn't gain popularity until tax laws changed in 1986 and gave employees greater tax advantages.

The purpose of the code was to allow employers to have a separate written plan for their employees that provided them with an opportunity to receive certain fringe benefits on a pretax basis. All plans are strictly regulated by the IRS and must meet specific requirements and regulations that can and do change. The written plan must specifically describe all benefits and establish rules for eligibility and elections. Because of the stringent guidelines, many employers offering these plans have them administrated by an outside source.

How do Flexible Benefit Plans Work?

Each payroll through a deduction from their paycheck, the participant contributes money to a Spending Account. This money is not taxed. The participant is then reimbursed from this account by their employer (or outside Administrator) for those health care expenses not covered under a health insurance policy or by a health care provider.

The participant's expenses are reimbursed in full up to the maximum amount of their annual election at the time the claim is submitted. The expenses must be incurred during the plan year while they are covered by the plan. Any funds left in the Spending Account at the end of the plan year will be lost.

These plans run for a year at a time. At the beginning of the plan year, the participant chooses how much they are going to contribute - called the annual election. A participant can have two Spending Accounts; one for themselves (called the Health Care Account) and one for their dependents (called the Dependent Care Account). The Health Care Account and the Dependent Care Account are treated separately and so funds from one account cannot be transferred to the other account.

In deciding what the annual election should be, a participant should look at their health insurance deductibles and copayments, as well as uninsured medical, dental, vision care and hearing care expenses. The uninsured expenses covered must be "medically necessary" as determined by a doctor or healthcare service provider.

You may see some generalized expense categories listed when people talk about what expenses are covered. These may include things like:

> Non-reimbursed medical expenses resulting from charges due to hospital, physician, dental, orthodontia, vision, and prescription drug expenses incurred.

> Deductibles and copayments resulting from charges due to hospital, physician, dental, orthodontia, vision, and prescription drug expenses incurred.

Why are Flexible Benefit Plans Good?

From the employer's point of view, you are offering your employees an added benefit and helping them to save money on their taxes.

From an employee's point of view, you save taxes and you pay for health care expenses not covered under a health insurance policy or by a health care provider with money that has been set aside for you through payroll deductions. Thus, when these expenses come due, you don't have to pay for them from your daily cash flow.

Without a 125 Plan, you have no pre-tax deductions for premiums, child/dependent care or other out-of-pocket medical expenses.

So say you made $26,000 a year. Without the pre-tax deductions allowed by a 125 Plan, you would be taxed on the full $26,000. At a Federal Income Tax Rate 15%, a State Income Tax Rate of 3% and a Social Security Tax of 7.65%, you would be taxed a total of $6,669.00 on your income of $26,000 if you didn't have a 125 Plan.

If you had a 125 Plan and paid $1,000 for premiums, $5,000 for child/dependent care and $500 for out-of-pocket medical expenses for a total of $6,500 you would be taxed on $19,500 instead of the full $26,000.

At the same tax rates mentioned above, you would be taxed a total of $5,002.00 if you did have a 125 Plan.

And don't forget without a 125 Plan that you would still have those premium, child/dependent care or other out-of-pocket medical expenses of $6,500 to pay.

So what does that all mean to your net take-home pay?

Without the 125 Plan:

$26,000 = $12,831

With the 125 Plan:

$26,000 = $14,498

By participating in a 125 Plan, you would bring home an extra $1,667.

Hospital's Role in Surrogacy


Surrogacy is generally a medical practice, even though some legal squabbles could also turn it into a legal matter. The general bulk of the procedure is purely medical. The role of the intended couple, the surrogate, and the agency has been discussed at length in other articles already. But we often overlook what the role of the hospital is in this whole process especially in the delivery process.

The hospital plays a very major role in every surrogate pregnancy. Although the IVF (In Vitro Fertilization) is done in some clinic, most of the testing and ultrasound scanning would probably occur inside the hospital. This is not the biggest part the hospital plays in a surrogate pregnancy though. That occurs when the surrogate mother is about to deliver the baby.

There should already be word sent ahead of the arrival of the surrogate mother and that this is a surrogate birth. That should already have been handled by the surrogacy agency that handled this woman's pregnancy. The hospital would then seek to inform the intended parents of the impending birth just to make sure that they know. Although, in most cases the surrogacy agency itself is responsible enough to do this on their own and the intended couple also arrives with the surrogate mother. This is because the intended couple would usually want to take part in their baby's birth. Even if it is not the mother herself who is giving birth, the experience can still create a lasting memory and a special bond between mother and child.

The delivery should then be attended to just like any other delivery, with a full team of doctors, nurses, and other medical professionals to assist in the whole procedure. Once the baby is born it would be the hospital's responsibility to put identifiers to the baby. This is a little bit more complicated than usual births since there are 2 sets of parents; the surrogate mother, and the intended couple. Hospitals have created a method to deal with these new set of challenges. They created a method called double banding. Instead of tying 1 band around the baby's arm, they tie two. One is for the surrogate mother and other for the names that appear in the pre-birth order, which has been secured by the surrogacy agency from the court.

The baby is then taken away into the nursery so that it can undergo the proper tests necessary for a newborn baby. After this, follows a more or less standard procedure with the surrogacy agency taking care of the extra paperwork needed to facilitate the custody of the baby and taking care of the surrogate mother to make sure that all her needs are met.

Having a surrogacy pregnancy can be long and difficult, but everyone has a role to play in this struggle to provide a loving couple the child that they have always wanted. The hospital is just another member of the team that made all this possible.

Overeating While Pregnant


Pregnant women understand the importance of taking good care of themselves and watching what they put into their bodies during their pregnancy. We are cautioned to take great care to avoid alcohol, bad foods, drugs, cigarettes, certain prescription medications and anything that could cause problems for the baby. Many women, however, use being pregnant as an excuse to overeat unfortunately. You have probably heard or even used the expression "eating for two." The truth, though, is a little different. When tempted to overindulge, remember that you are not actually eating for two full sized adults and overeating during pregnancy can lead to a number of issues.

There are many health issues that women experience during pregnancy and overeating can contribute to making some of them worse. Back pain is a common problem that many expectant women deal with and one that is certainly exacerbated by gaining an unhealthy amount of weight due to overeating. Similarly, heartburn is something a lot of pregnant women are susceptible too and again overeating can make it much worse. It is often difficult to get a good night's sleep and if your stomach is churning from indigestion caused by eating too much then tossing and turning is virtually guaranteed. The delivery and losing the excess weight following delivery can both be made more difficult by overeating as well.

So what is a normal amount of pregnancy weight gain and what can be considered overdoing it? Most doctors and dietitians cite twenty five pounds as the expectation for the average woman. If you are underweight to start you may gain more and likewise if you are overweight when you get pregnant you may gain less. During your first trimester, you do not need to take in a great deal of additional calories. You may only gain five pounds during your first trimester. During the second and third trimesters, one pound a week is normal and two pounds a week is acceptable. Gaining more than two pounds a week could be an indication that you are overeating.

Some women experience cravings while others report feeling hungry almost constantly. Neither one of these feelings are a reason to overeat, however. We all have different dietary habits and metabolisms, but these issues can be combated by making slight changes to the way we eat. Eating more frequent smaller meals and getting some regular, light exercise are ways to avoid hunger. You can still eat the foods you crave and work them into a healthy eating plan as long as you do not overindulge.

We all know how important it is to watch what we put in our bodies while we are pregnant. Overeating though is something that all too often is overlooked. Talk to you doctor about eating healthy and expected weight gain. Take care not to overdo it so your pregnancy and your delivery can be as comfortable and as smooth as possible.

Find a time when the baby is happy and relaxed. It's no use planning a massage when your little cherub is hungry, exhausted or screaming his or her head off. It's best when they wake up from a nap or are in a good mood before a feed.

Choose a quiet, warm place - it's recommended that the room is roughly 75 degrees Fahrenheit. Don't schedule a massage in a cold kitchen, or when there are screaming toddlers around. The goal is to make baby feel happy and secure. Get it?

Clean and warm your hands. Avoid using a heavily perfumed soap or hand lotion.

Is Your Pregnancy Provider Mother Friendly?


If you are planning to breastfeed, birthing your baby in a hospital with a Baby Friendly designation can be beneficial when compared to birthing in a hospital that is not Baby Friendly. In spite of the benefits of Baby Friendly, however, it is important to realize that this designation gives you no information about a hospital's labor and delivery practices. Many families unfortunately get lured into thinking Baby Friendly somehow translates into Mother Friendly. The truth is it doesn't. Baby Friendly does NOT mean Mother Friendly.

The focus of the Baby Friendly Hospital Initiative is to promote BREAST FEEDING. Nothing in the steps to becoming a Baby Friendly hospital say anything about labor or birth. Fortunately, in 1996, the Coalition for Improving Maternity Services (CIMS) along with 31 individuals and 26 organizations ratified the Mother-Friendly Childbirth Initiative (MFCI). The CIMS "mission is to promote a wellness model of maternity care that will improve birth outcomes and substantially reduce costs."

The MFCI has identified ten steps that make maternity care Mother Friendly and promotes normal birth. These ten steps include, among other things, offering all birthing mothers unrestricted access to the birth companions of her choice; access to professional midwifery care; and the freedom to walk, move about, and assume the positions of her choice during labor and birth (unless restriction is specifically required to correct a complication).

Additionally, to be Mother Friendly, a facility would avoid routinely using practices and procedures that are unsupported by scientific evidence; they would educate staff in non-pharmacological methods of pain relief, and they do not promote the use of analgesia or anesthesia unless required to correct a complication. It's important to recognize that even with a hospital designation of Mother or Baby Friendly, your specific birth attendant may not agree with or practice in accordance with the designation. Your birthing experience depends on it.

How Ultrasound Works


Ultrasound is a form of energy and radiation, but not ionizing radiation. Like other sources of energy, it occurs in sound

waves which move at frequencies higher than the human ear can hear. The ultrasound waves are emitted at 2 to 4 megahertz which is a higher frequency than AM radio. The waves bounce off of the internal organs and are interpreted by the scanners which produce a picture on a TV-type screen. The transducer or scanner produces the sound waves and sends them out. It also reads the bounced waves. There are three types of ultrasound. Ultrasound scanning device, Doptones and External Fetal Monitors.

For maternity care, the ultrasound probe is placed over the abdomen. Sometimes a trans-vaginal or trans-rectal probe will be used and this is shaped so that it can be inserted into the vagina or rectum. The scanning devices that make pictures deliver pulsed waves. These devices are usually used for a short period of time. Real time ultrasound is most frequently used. This type of ultrasound makes many pictures of the image being scanned, similar to movie film, and can show fetal activity and how the baby is positioned and the number of babies visible.

An extremely uncommon type of ultrasound is the Doppler. It gives sound only as the sound waves are reflected from the baby's heart. Some Dopplers have earphones that only the practitioner can hear. Others use an amplifier so that the heartbeat is heard aloud. Electronic Fetal Monitors are used most frequently in a hospital setting during labor and delivery. Most infertility treatments are self paid: the majority of group health insurance plans provide little or no coverage for these procedures, leaving couples with lots of un-reimbursed medical expenses. A flexible spending account provides the best tax savings, and an often overlooked loan feature. But the loan feature will lose it's appeal in 2013, so move fast if you are looking at IVF or other infertility treatments.

A flexible spending account (FSA) provides immediate tax savings for un-reimbursed infertility expenses. You use pre-tax dollars to pay for fertility drugs, diagnostic testing, IVF cycles, etc. Anything not covered by insurance can be paid for using this account. Unlike taking the deduction at the end of the year on IRS Form 1040, a flexible spending account has no itemized deduction threshold, so your tax savings are much better.

Flexible spending accounts also provide an employer sponsored loan. Up until 2013, your employer sets the amount you can contribute into your FSA. Some set small limits, while others allow up to $10,000 or more. The key feature is that your employer must immediately reimburse any qualifying expense, no matter what you have paid into the account.

For example, suppose in December you elect to contribute $10,000 annually to your FSA because you are planning to undergo IVF in January. Your company's FSA plan year begins January 1 of the new year. Your employer funds the full FSA election in January because you had a qualifying expense. You then have eleven months to repay your employer via pretax payroll deductions. Your just got an interest free loan that actually cut your IVF costs by one third or more depending upon your income and tax status.

Uncle Sam can't tax it because you don't get back a dime more than you put in, meaning you didn't actually "earn" any interest. So even if your internal rate of return is just 6%, that's still a terrific net rate.

One more thing before we wrap this up: Don't buy ROP unless you're committed for the long haul. Let your policy lapse half way through the term, and you might get back 20% of your premiums in surrender value. Hold it until the end of the term, and get back 100%. There are mostly no side effects apart from sense of discomfort and occasional headache. Let's hope with more research the treatment will become more affordable.

    - Obviously, the antidepressant drugs are here for a long run as well. There will be more new pills, hopefully with less side effects.
