Women's hygiene, a very important matter which needs to be addressed. A subject, which women need to learn about. I have been learning about the different types of women's hygiene available, and believe me, there is a lot that us women don't know. For example, did you know that there are about 13 different types of sanitary pads available?, Not only that but most places which stock women's hygiene also do maternity pads and incontinence pads.

See there are many things which women do not know about their personal care, like for instance, what suits them best. A Slender Maxi Pad, is thicker than a regular, however it is slimmer, for those with light to medium flow.

See you don't have to wear, the first one you pick up, read the label. What suits YOU best? Which would you prefer? We are all different.

Now, Ultra Pads come in regular, Super, Super plus, and long, also all of these may or may not have wings. Hunt around, find the one which you will be more comfortable with. Now I am not going to go into the ultra pads, because I guess that you can probably guess which one's are which. However I will say one thing, regular is ultra thin, super is thin, super plus is thicker than super, and long, is for night times and for extra protection when you need it most.

The current American College of Obstetrics and Gynecology guidelines state that thyroid functions should be checked only in women with a personal history of thyroid disease or symptoms of thyroid disease. It is NOT universally recommended to test all pregnant women even though there are cases of women who have disease that do not have symptoms (subclinical cases).

How does maternal thyroid hormone effect the fetus?

The fetal brain is completely dependent on maternal thyroid hormone until about 12 weeks gestation. At that time, the fetus is able to manufacture its own thyroid hormone in conjunction with the maternal hormone that crosses the placenta. Diminished levels of thyroid hormone in the mother impair fetal brain development. Elevated levels can also cross the placenta and cause excessive production in the fetus. (Graves disease.)

What is hyperthyroidism?

The thyroid is an endocrine gland located in the neck that controls metabolism. It receives a message (TSH) from an area in the brain called the pituitary which releases thyroid hormone (T4).

When the gland produces more hormone than it is supposed to, hyperthyroidism is diagnosed (elevated thyroid hormone T4 and low TSH.) This can occur in about.2% of all pregnancies. The most common form of the disease is Graves disease where certain antibodies are made by the body that stimulate thyroid hormone production. Other causes can be multinodular goiter, subacute thyroiditis, an extra thyroid source of hormone production (certain tumors of the ovary or pituitary), thyroid adenoma.

What are the symptoms of hyperthyroidism?

What are the risks to the mother and the fetus if hyperthyroidism is left untreated?

If left untreated, hyperthyroid can cause:


  • preterm delivery

  • severe preeclampsia

  • heart failure

  • fetal loss

  • low birth weight infants

  • stillbirth

  • fetal hyperthyroidism

How do you treat hyperthyroidism in the mother?

A classification of drugs called thioamides are used to treat hyperthyroidism.


  • PTU

  • methimazole

These drugs prevent the manufacture of the thyroid hormone by preventing a needed substrate iodine from attaching to the thyroid molecule and it also blocks the the manufacture of of another active form of the hormone T3.

These drugs do cross the placenta and can effect the fetal thyroid, although it is generally transient. Generally, these drugs are safe to use in pregnancy but rare side effects of the drug can include fever, sore throat, hepatitis, rash, nausea, loss of taste and smell, loss of appetite and a very serious and rare side effect called agranulocytosis (less than 1%) which is an abnormal condition of the blood characterized by a severe reduction of white blood cells (fever, prostration and bleeding ulcers of rectum, mouth, and vagina.)

Infants must be observed carefully after birth with mothers on antithyroid medication since newborns have been known to have neonatal hypothyroidism and goiter in mothers who have been treated. Babies are ultrasounded during pregnancy looking for fetal goiter and growth problems which can present problems at delivery due to the hyperextension of the neck.

It is generally considered safe to breast feed on these medications.

Other drugs used to treat hyperthyroidism are beta-blockers (propranolol) which act to reduce the rapid heart rate that can occur. Side effects from this drug can include growth retardation in the fetus, fetal bradycardia (slowed heart rate) and hypoglycemia in the infant (low blood sugar).

Radioactive iodine is never used in pregnancy since it can ablate the fetal thyroid. A patient was treated with radioactive iodine prior to becoming pregnant, should avoid becoming pregnant for at least 4 months. If all medications fail, or allergy to the medications exist, thyroidectomy, or surgical excision of the thyroid is recommended.

What is subclinical hyperthyroidism?

In about 1.7% of women there are asymptomatic women with normal thyroid hormone but a low TSH. This condition generally has been found to have no effect on the pregnancy since it is the maternal T4 level that is critical for fetal brain development, regardless of what the TSH level is. However, these women should be observed for osteoporosis, cardiovascular morbidity and progression to overt disease or thyroid failure in the future.

What is thyroid storm?

Thyroid storm is an acute obstetrical emergency that occurs in about 10% of women with hyperthyroidism. Symptoms include a change in mental status, seizures, nausea, diarrhea, and cardiac arrythmias. Patients are placed in the intensive care unit for constant monitoring and observation since there is a high risk of maternal heart failure. Thyroid storm can be precipitated by an acute surgical emergency, infection, diabetes. anesthesia, and noncompliance with thyroid medications. In addition to the usual treatment of hyperthyroidism as described above, steroids are commonly given.

Can thyroid disease present itself right after delivery?

About 6 to 9% of women with no history of thyroid disease can present with disease after delivery, generally within the first year postpartum. This is common in women that have previously known thyroid antibodies that are not activated until after the delivery, or women with a strong family history of diabetes or other autoimmune disorders. Most women have transient hyperthyroidism which then converts to hypothyroidism requiring treatment. About 77% of women will completely recover but 30% will continue with thyroid disease permanently. Many women that recover will develop this disorder again with subsequent pregnancies.

Exercise During Pregnancy - What's Safe and What's Not Safe?


If you weren't fit before you became pregnant, don't give up! Exercise during pregnancy doesn't have to be ultra-strenuous. Begin slowly and build gradually as you become stronger.

When doing pregnancy exercises, keep your heart rate under 140 beats per minute and avoid overheating, especially in your first three months. Start gradually. Even 5 minutes a day is a good start if you've been inactive. Add 5 minutes each week until you reach 30 minutes at one time.

The Very Best Exercise During Pregnancy-Swimming

Swimming is really the safest and best pregnancy exercise, as it does not have any impact on your body, as jogging or walking does. You can reach a high level of heart rate, which strengthens your cardiovascular ability and gives you a welcome buoyancy (floatability or the feeling of weightlessness).

Swimming is a safe cardiovascular exercise that will most likely not cause overheating.

If you can't swim, join a water aerobics class. Water aerobics are great pregnancy exercises as again, the water has no impact and is very safe on your stomach.

Walking

Running

Aerobics

If you already participate in aerobics, you will most likely be able to continue; however, you should speak to your health care provider before you begin. Avoid jumping, spinning and leaping.

Keeping your balance can sometimes be difficult, so you'll want to be careful as you grow. Do not exercise lying flat on your back for extended periods of time, and don't even lie on your back after 25 weeks.

If you do choose to do aerobics, just make sure to avoid becoming extremely winded or exercising to the point of exhaustion.

Kegel Exercises

Pregnant women who perform Kegel exercises often find they have an easier birth. The best thing about Kegel exercises is that they can be done anywhere, and no one knows you are doing them. Just squeeze the muscles that you use to stop urinating for a few seconds and do this 10 times per day.

Bicycling

The best thing about biking is that the bike supports your weight, so there is less stress on your body. Using a stationary bike is great exercise because you have less of a chance of falling.

Dance

Dancing is fine as long as you do not do a lot of leaping, jumping or spinning.

Stair Climbing Machines

Stair climbing is a great way to raise your heart rate and improve cardiovascular endurance. Just hold onto the side rails so you don't fall.

Yoga

Most forms of yoga will be safe for you and your baby, as long as they are not excessively rigorous. Some yoga instructors offer special classes for pregnant women. Avoid lying flat on your back for extended periods of time and try not to over stretch.

Warning Signs

Most health care providers advise pregnant women to avoid any exercise during pregnancy after the first trimester that requires them to lie flat on their backs.

As swimming poses no impact problems and you don't get overheated like you do with other exercises, it is the best pregnancy exercise to do right up until your due date. I was swimming two days before I delivered!

Whatever you choose to do, do it consistently and with care. Pregnancy exercises are very important to your overall health, your cardiovascular strength and will increase your circulation, contributing to an overall healthy pregnancy.

Independence, Motherhood and Feminism in Kate Chopin's The Awakening


"Awakening" (In Chopin C. "Awakening and Other Stories" Random House, New York, NY: 2000) involves issues of feminism whereby Edna Pontellier, the center of the American Creole/New Orleans story, often and perplexedly reflects on her role of mother-of-two who is married to a well-off and often traveling (for business or pleasure) brokerage-business husband (Leonce Pontellier). The book garnered a lot of controversy for many decades after it was written in 1899, but eventually became and has endured as Kate Chopin's most famous work.

The community views Leonce as the epitome of the ideal husband, for Leonce greatly adores and provides for wife and children, he is quite consistently concerned about the welfare and happiness of his household. Yet Edna does not look at Leonce as her choice of husband, she says their marriage was accidental, that as she was growing up there are particular men that came around her that she would have wished to take her hand. Leonce is disciplined, insistent and low-toned, often dissatisfied about Edna's attention to the children and other household issues, more so because he is often away on business and Edna has a lot of help, Leonce sometimes causes Edna to walk off and cry.

Perhaps Edna was the precursor of the modern era American woman...one who is prevalently independent (or at least longs to be), one who has more power in making decisions about what she prefers, one whose identity is not predominantly defined by wealth, looks, family, husband, or children. In her state of psychological disillusionment ("An indescribable oppression, which seemed to generate in some unfamiliar part of her consciousness, filled her whole being with vague anguish" [179]), Edna's love for the two boys she gave birth to is uncharacteristically distant, the mother's instinct seemingly weak, the kids have more fondness for their father. "If one of the little Pontellier boys took a tumble whilst at play, he was not apt to rush crying to his mother's arms for comfort....Mrs. Pontellier was not a mother-woman..." (181).

The author Chopin hence paints a picture of a soul plagued by a mixture of feminist and psychological issues. It is to be remembered, that even in this age of feminist liberation, providing men who love and care for their wives and children are still held in high esteem and are still in high demand. Chopin implicitly displays that female emancipation and longing can be of numerous forms. Further, Chopin often compares and contrasts main characters in terms of their beauty and body textures/ forms, illustrating that this issue has been strong in the United States for centuries.

"[Edna] was rather handsome than beautiful...face...captivating by reason of a certain frankness of expression and a contradictory subtle play of features" (174). Chopin writes of Edna's companion and friend Adele Rattignolle (as somewhat contrasted with Edna) that, "...her beauty was all there, flaming and apparent...two lips that pouted...the grace of every step, pose, gesture... (182). Many writers have noted that Kate Chopin was not a suffragist and did not join any feminist movements; and indeed, many feminist writers reduce the value of the aesthetic features and comparisons of women, aspects that can remind of Hollywood vagueness.

Eventually, Edna wants to leave her family big house and settle "...in a four-room house along the corner" (294). A woman who had great difficulty at learning how to swim, one who is still sophomoric at it, Edna will ultimately find disturbing comfort in introspectively walking alone to the sea, in taking off all her clothing, reflecting on her life and swim into and allow herself to be swallowed by the sea, a fatal blow to the self-possession and psychological emancipation that she sought. Edna was born a reserved child, she was miserable and felt stifled, her marriage and sketches did not reach the standards she wanted them, there was something lacking in her "ideal, loving family; her comfort and big house," the lustful affair with the lad Robert the conspicuous and philandering son of Madame Lebrun the property owner was full of gaping holes and would not last. Edna did not fit in the conventional mode of the beginning of the 20th Century American dream woman. She instead provides a window into what the woman, over the numerous decades to come, would perhaps evolve into as the individualist and the emancipated female.

In "Awakening," Edna Pontellier is supposed to be one of the most fortunate women in the world. She has a present, fidel, hardworking and earning, capable, loving husband, who sired with her two male kids, she does not have to go outside of home to work. Yet, she is dissatisfied with her status quo, a rich homely situation that many women crave to be in. The narrator is telling us that women are complex persons, each of individual personality, one size does not fit all; needs and interests, and ambitions of each woman vary. Edna is horrifyingly distant from the two boys she gave birth to. "If one of the little Pontellier boys took a tumble whilst at play, he was not apt to rush crying to his mother's arms for comfort....Mrs. Pontellier was not a mother-woman..." (181). But indeed women (and female animals) have been known to neglect and even kill their children, reasons for which vary from psychological to dislike for tending to offspring. Women have been known to chase down and drown their children in bathtubs (Andrea Pia Yates, in 2002, drowned all five offspring in a bathtub; she has had a history of post-partum depression and psychosis), or let a car-full of their children roll into a river and get them drowned (Susan Smith, in 1994 did this to her two sons).

Edna Pontellier had two sons, she drowns herself in the end. Women have been known to leave their enviable husbands and children in a comfortable life, and fall for top-notch criminals who are locked up in prison. Edna mentions wanting to leave her comfortable home and enviable family, and living in a smaller habitation down the street "...in a four-room house along the corner" (294). Post-partum depression has been mentioned about women, and in Edna's case it seems to have become indefinite. Many women, even in the contemporary times long for biological or at least adoptive motherhood. Still, there are those who opine their biologically begetting children as one of the most unfair ways a woman is exploited as a painstaking residence of a baby during gestation. Human gestation is a trying experience, compared to that of most other species (consider squirrels and rabbits). Human fetuses comparatively have many defects, miscarriages are common. At the same time the man does not have the burden of carrying the child to childbirth and keeping a sleep-ridden eye on the child. The fathers of the child sometimes wander away, abandoning their offspring.

Edna portrays that a woman can want much more than a family of children and a husband, perhaps she was a lesbian who had not discovered herself as one. She was at least discontented with her husband Leonce who comes off as conventional, disciplined, and inflexible. This discontentment is understandable...it happens. But why the distance from her offspring, and then the lustful interest in the young man Robert, eventually adultery? Edna will always be an enigma! Perhaps Edna suffered from multiple-personality disorder, something psychological irked her. Perhaps she longed to be the independent free woman, one who had the freedom to love or have sex with her choice of person, the precursor of the 20th and 21st Century independent and upstanding woman free to express her sexuality and stick to her preferences. Edna, many times in subtle ways, brings forth into question, feminism in the context of individuality, sexuality, marriage, freedom and choice, reproduction and child-rearing, spousal attachment and power, and the context and role of marriage in a woman's life. Edna brings it out that each female is of unique individuality, of personal talent and likes that beg to be fully uncovered so she need not be comfortable with how society compartmentalizes women, more so as wives, mothers, home-makers, and as cherished articles of beauty and ownership.

Understand Cat Pregnancy and How you can Help your Cat through this Special Time


Unneutered cats are very prolific breeders and can produce three litters a year when left to their own devices. The female cat is usually able to breed from as young as six months old, sometime even younger.

The signs of a cat in heat or to use its medical name Oestrus, can be quite subtle and can be easily missed by cat owners. Although there will be some bleeding during this time, it is a lot less noticeable than in a human and if the cat is allowed outside may be missed completely. Usually the first indications that a cat is "on heat" are changes in behaviour. Often the female cat will become overly friendly towards everyone and will rub herself around your legs more frequently than usual. She will also roll around on her back more often, make unusual yowling noises and will want to urinate more often, which helps to spread her scent around.

If your female cat does have contact with complete male cats during this time, it is likely that she will mate with all that she encounters. This explains why a litter of kittens may contain many different fur colourations and markings, as they may all have different fathers.

The gestation period for a cat is between 60 and 65 days and the first signs that your cat is pregnant may not be evident until the third week of pregnancy, when her nipples will start to become enlarged. By week four the pregnancy should become visible and it during this time that a vet will be able to feel the kittens through her abdomen.

During your cats pregnancy it is important that your cat is fed top quality cat food, choosing one that is formulated for kittens is a good choice as it is higher in protein and calcium. However over feeding should be avoided, as over weight cats can suffer complications during birth. Extra vitamins may be beneficial to your pregnant cat, but supplements should not be given without the approval from a qualified vet.

As the birth draws near, the queen will start to look for a place to have her kittens. She will probably want to choose a quiet secluded familiar place that is warm and secure. As an owner you can help choose this place by placing a cardboard box lined with newspaper in a quiet warm room. Food, water and a litter tray should also be placed near by to encourage your queen to make this her birthing box.

One of the first signs that your cat is preparing to give birth is that her appetite will decrease and she will either begin to want to be on her own more than usual or she may seek out human affection to help reassure her. Also her mammary glands will become very swollen and may start to produce milk a few days before the actual birth.
During this time it is important for the owner to keep a close eye on the cat and to watch out for any signs of distress or illness. Any signs of a temperature or if the cat has not eaten at all for 24 hours, should be investigated by a vet.

Cats are normally able to give birth without any problem completely by themselves. The process appears to be practically pain free, and apart from the actual straining, the cat seems unperturbed by the whole situation. Human intervention is rarely needed, even dealing with the umbilical cord and fetal membranes can be left to the cat.

After the first kitten is born it may not be for another hour or so before the next kitten is born. However it is not unusual for successive kittens to be born 12 to 24 hours after this first wave of kittens. It is therefore important not to let your cat out or try to handle them for at least 24 hours, to make sure that the process is completed. Reassuring strokes and tickles will be enough to comfort the cat and reassure her that all is well. Fresh water, food and a litter tray should be provided close by.

Mother and kittens should be allowed to be left alone to bond and the handle of the kittens should be minimal for the first 24 hours so that our scent does not mask their own and their mothers.

My Personal Experience With Caregiver Stress


I began my career in a cancer center. I worked in the Medical Oncology Unit, otherwise known as Med/Onc. Prior to working with individuals with a cancer diagnosis, I worked with seniors and had a great deal of experience with death and dying. At the end of the day, I was able to put my work aside and go home and not deal with any of the sadness and human suffering I encountered on a day to day basis. I thought that working with the elderly population would prepare me for my work with oncology patients.

As a young social worker, I thought I had it all together. I thought I knew how to juggle all of the balls that were being thrown my way. I would go to work on a daily basis, meet all of the demands of the day, and make sure all of my patient's needs were taken care of. I came face to face with real human suffering. I came face to face with mothers losing their children, children losing their parents and the unfairness called "cancer". Despite all that I encountered, I thought I was able to handle it as I did in the past, I thought I could come face to face with death and put it aside at the end of the day.

I was very proud of the work that I did and to be a part of such an amazing organization. As proud as I was and as much as I enjoyed the work that I did, I always carried with me an overwhelming feeling of dread. This was something that I could not shake. I did a pretty good job of masking my feelings, so good in fact, that I hid them from myself. My superwoman complex kept me from being truly real with myself. I was too proud to admit the truth...I was sinking into a depression. I did not realize it because it was insidious and I had lived with the symptoms for so long that I just thought this was the way I was supposed to feel when encountering human suffering. I never talked about it with anyone, because I could not put my feelings into words. No amount of education and past experience prepared me for what I was facing. It was not until one day that I went to see a movie with a close friend that I finally had some insight into my situation. We went to see a romantic comedy called "the rolling stone". I always avoided dramatic or sad movies because I did not want to feel sad when I was home. It was a defense mechanism I used in order to keep my wall of denial up and my defenses safe. (**spoiler alert**) There was a part in the movie where one of the main characters was diagnosed with breast cancer and subsequently dies. I immediately began sobbing uncontrollably. It was such an exaggerated response and as me and my friend sat alone in the movie theater, I was ashamed of myself and my emotional response. The following week I had a meeting with my immediate supervisor, where I informed her of what happened during the weekend. She was so supportive and gave me some invaluable insight...I was suffering from caregiver stress.

Caregiver Stress is stress caregivers experience when providing care to another person suffering from some type of illness. Caregiver stress is not limited to individuals caring for a loved one. This definition can also include medical professionals and others working in a medical field providing hands on care and counseling to individuals.

This was the first time where I put my pride to the side and really began to take a long, hard look in the mirror. For so long I thought that expressing my true feelings would make me appear weak or somehow undermine my clinical training. When I finally put my guard down, I was able to take active steps in addressing all of my symptoms. As a clinician it is so much easier to have empathy for others and to assess situations and provide good clinical counseling. In order for me to be a better clinician I had to be real with my own insecurities, inadequacies and emotional struggles. I had to be real in how jaded the job was making me. In my time alone, I would often time struggle with God and question why good people have to suffer and be honest with my own physical symptoms of depression.

I began my journey to healing. I began to really look at caregiver stress and its effects. I saw that I was experiencing classic symptoms.

As I understood some of my own personal feelings, I was better able to cope with all of the human suffering around me. I was kinder to myself and as a result, I was better able to provide good clinical interventions. As the feelings of caregiver stress multiplied I was exceedingly overwhelmed and not truly able to give the individuals I was working with the best of me. As I increased self care and made positive steps to address my own inadequacies and sadness about what I encountered on a daily basis, I was able to provide better patient care.

This new found awareness also continued in my personal life. While working I became pregnant with twin daughters. Of course my husband and I were delighted but no one could have prepared us for life after twins. Needless to say, the first 6 months were very difficult and I found myself overwhelmed, sleep deprived, anxious and so forth. Not to mention, I also experienced some postpartum depression. My past experience with caregiver stress gave me tremendous insight into my condition. I applied some of the same techniques and as a first time mother. It's hard when you are in the "thick of it" to put words to your feelings. We often times have more empathy to those around us and don't cut ourselves enough slack.

So often we may consider taking time for ourselves as being "selfish" and we do not make the time. I say to you, make the time. If you do not care for yourself, then you cannot care for others in the best way. Even the best intentioned people can "loose it" when they are overwhelmed and exhausted. Be just as kind to yourself as you are to others.
