Premenstrual Syndrome! That's right! It's that old friend, PMS, the subject of so many jokes and putdowns. You know - that "catch-all" phrase that, according to "so-called comedians," covers all the "imaginary" problems that women "invent" as excuses to spend the day on the couch "watching TV and eating bonbons."

Let me assure you that, while it is true that doctors and researchers have not found a specific lab test that can identify PMS, this disorder is definitely neither "imaginary" nor is it "invented." Serious medical research has been going on for years to identify the definitions, diagnosis, causes, and cures for this disorder. Here are two of the resulting definitions:

1. Dr. Ellen Freeman of University of Pennsylvania Health System describes PMS as a "cluster of emotional, behavioral, and physical symptoms that have a cyclic pattern related to the menstrual cycle, becoming severe in the week or two preceding menses and abating with menstruation."

2. The American College of Obstetricians and Gynecologists defines PMS as "the cyclic occurrence of symptoms that are sufficiently sever to interfere with some aspects of life, and that appear with consistent and predictable relationship to the menses."

How do you know if you or a loved one has PMS, if there is no real test for it? Across the board, researchers and doctors agree that the identifying factor overall in this disorder is the fact that the symptoms stop once the menses begin or once a woman becomes pregnant.

You should know that doctors have identified two serious forms of PMS that have effects far beyond those of normal PMS symptoms:

1. "Postpartum Depression" is a severe depression that some women experience after giving birth. Women with this disorder suffer a severe let-down from the high pregnancy-levels of hormones, and, because of the disruption to their nervous systems, they may actually harm themselves or their babies.

2. "Premenstrual Dysphoric Disorder" (PMDD) brings about serious and disabling depression, anxiety, tension, and angry irritability, intense mood changes in the week or two before the onset of menses, and women suffering from this disorder exhibit at least five of PMS's physical symptoms as well. Like regular PMS, PMDD symptoms disappear with menses.

You may have heard that doctors find it helpful for women to keep a daily diary of symptoms throughout the month when they are attempting to diagnose PMS. What should you look for when keeping a daily diary or considering the possibility that you or a loved one has PMS? Well, PMS carries over 150 possible symptoms that affect women in both the physical and the emotional realms in varying degrees.

Physically, symptoms can range from light, as in headache, fatigue, and slight abdominal swelling, to severe, as in migraines, severe cramping, and an inability to function normally. Emotionally, you may experience symptoms relating to anxiety (irritability, irrational crying, or emotional swings), depression (withdrawn, fearful, or altered libido), cravings (for sweets, dairy, or alcohol), and fluid retention (headache, breast tenderness, abdominal bloating, and weight gain).

I can tell you that although doctors still have not determined the exact cause of PMS, they agree that hormones and neuro-chemical changes within the brain are prime suspects. Estrogen hormones, for example, can contribute to increased brain activity, retention of salt, and drops in blood sugar, because it has a central neurological effect on the brain.

What do you do if you realize that you or a loved one suffers from some level of PMS? Obviously, if the symptoms are severe, you should seek your doctor's advice. Doctors today may prescribe either psychiatric (psychotropic drugs, tricyclics, tranquilizers, or selective serotonin reuptake inhibitors), medical treatments (anti-estrogenic hormonal medications), or a combination of both for severe cases.

However, if you believe that your symptoms are manageable, or if they are intermittent, here are a few tips that might help you alleviate some symptoms.

- Eat six, small, high in complex carbohydrates, low in simple sugars meals at three-hour intervals to help keep energy levels and blood glucose levels steady. Complex carbohydrates are found in foods eaten in their natural states like oatmeal, cornmeal, barley, and wheat germ, or in foods like pastas, brown rice, root vegetables like potatoes, whole grain breads and cereals, etc.

- Reduce caffeine, alcohol, salt, fats, and simple sugars to help diminish fatigue, tension, depression and water retention bloating.

- Add calcium, controlled levels of B6, B complex, magnesium, and Vitamin E to help maintain normal moods and reduce fluid retention, cramps, and back pain.

- Exercise three times a week for at least 20-30 minutes to improve mood, blood circulation, sense of well-being, and alleviate the build-up of tension and stress.

Get rid of all distractions for optimum use of your time. You do not have to live the lifestyle of a Spartan, but there are some times when the "technological toys" get in the way of real progress.

Lastly, reward yourself with quality time, when you have finished a project. Make sure that you take the time to enjoy your family, friends, and quality relationships. You are a Yoga teacher, who teaches the art of living, and you should also enjoy a balanced life.

Postpartum Depression Scale - The Postpartum Depression Scale Defined


It is common to confuse simple "baby blues" with postpartum depression, but it happens all the time. Baby blues are very common and do not interfere with the mothers' ability to take care of the baby or to go about normal daily life. However, postpartum depression can be extremely crippling and can stop normal life.

The depression itself can leave a woman emotionally fatigued and physically drained.

Following are some of the symptoms of postpartum depression, and should be watched out for:

* Having extreme mood swings and emotional fluctuations
* Being unable to sleep
* Being unable to enjoy your new baby
* Being unable to enjoy activities you did previously
* Being very fatigued
* Having fluctuations in appetite
* Feeling inadequate as a parent and spouse
* Having negative thoughts about yourself, and/or the baby
* Being suicidal, and/or thinking of harming the baby

There are times as well when postpartum depression can be severe enough that it can become postpartum psychosis. With postpartum psychosis, a new mother may become delusional, may become severely depressed, or may even start hallucinating.

Health centers in Edinburgh and Livingston have developed what is called the Edinburgh Postnatal Depression Scale; also called the "postpartum depression scale," this scale helps to detect possible postpartum depression in mothers.

If you or your health care provider suspects that you may be suffering from postpartum depression, you will be asked some questions from the scale, such as how long you have had the depression, how intense it is, any previous history of depression you have had, any substance abuse problems you have had, any marital problems you may be having, or any other discord or form of stress you may be experiencing now, so that conclusions can be drawn.

You will also be asked to take what is called a "postpartum depression scale test."

The latter is a questionnaire that has 10 statements, each with four possible answers. If you are asked to take this test, you will be asked to identify what you have felt in the past week, that just previous to the time you take the test.

If your score is higher than 92%, it is likely that you are severely depressed. Ideally, this test should be taken between six to eight weeks after you have given birth, and it should be completed by you (or the mother who has given birth, as applicable) without first discussing answers with anyone.

In addition to the Edinburgh Postnatal Depression Scale, there is also the PPD Screening Scale Test. This scale test has 35 items on it that you grade from a scale, ranging from "strongly disagree" to "strongly agree."

After the test is taken, the physician should be consulted so that proper decisions and treatment can be discussed and applied.

The Process of Lactation


Lactation is the biological term used for the production of breast milk. When a baby suckles at the breast, nerve stimuli are sent from the special receptor cells located in the breast to the hypothalamus which is located in the brain. The hypothalamus is then stimulated to activate the pituitary gland ninto producing two different hormones.

These hormones are called oxytocin and prolactin. The hormone called oxytocin is stored in the pituitary gland and acts on muscle cells. These muscles surround the milk glands where milk is produced. Oxytocin causes the muscles cells to contract in turn squeezing milk out of the glands into the ducts.

Prolactin is the second hormone which is involved in the lactation process. Prolactin is also produced by the pituitary gland and is released into the blood stream each and every time the baby suckles at the breast. When prolactin is released into the blood stream, it travels to the milk glands in the breast and stimulates the glands to produce milk. The level of prolactin increases in the mother during pregnancy, but milk production is suppressed by another hormone, progesterone, until its level drops off after birth.

Oxytocin causes what is known and referred to as the "letting down" of milk from the glands. This causes different sensations in different mothers. Some will report shooting pains, others report a tingling feeling and sensation. Some report no sensation at all. All are normal. After this process, the milk is then transferred from the breast to the baby by a combination of two processes:

* Active milk expulsion by the mother due to the let down reflex

* Active removal by the baby who works on the breast tissue with tongue and jaw to receive milk.

The more a baby feeds at the breast the more milk will be produced. The mother needs to bring the baby to the breast often so that her milk supply can keep up with the demands of her growing baby.

Once you are pregnant, there will be changes to your body. One significant change is the swelling of breasts. Women tend to get swollen breasts as early as 1 - 2 weeks into the pregnancy. As a result of this, you may find yourself experiencing headaches early in your pregnancy.

And of course, who can forget the food cravings of the pregnant woman. Of course it might not be a longing to eat odd combinations of food like sardines and chips, but many pregnant women feel a craving for some types of food. This craving could also last for the entire pregnancy.

Swollen feet, swollen ankles and arch and heel pain are the main causes of foot pain in pregnant women. Muscles and ligaments stretch and soften because of an increase in the ovarian hormone relaxin, which makes your feet more prone to ankle and ligament strains on a daily basis.

The choice of footwear can also be a factor in the cause of foot problems during pregnancy. A shoe with a heel of approximately 1.2in (3cm) is recommended as they will help to shift the weight further forward on the feet. This can help to reduce any pain or discomfort. Wide fitting and supportive shoes will minimise discomfort and help to avoid long term damage. Choosing a shoe with a strap can help the shoe to stay in place although fiddly straps can sometimes cause problems in later stages of the pregnancy when you may struggle to reach your feet.

If high heels are a must they should only be worn occasionally or when you know you will be able to sit down through the day. On a daily basis, a supportive shoe should be worn.

When shopping for shoes try to shop later in the day as feet can swell throughout the day. Ensure that the shoes fit well before you leave the shop as it is a common myth that you can break in a pair of shoes - this is not true. Shoes with a round or square toe are recommended. If you are buying boots, make sure that there is room in the calf area as they calves can swell. Choose a boot with a side zip fastening as they will be easier to take on and off - this will be much more appreciated towards the end of your pregnancy!

Keeping active is also recommended to help reduce the risk of cramps and swelling. Do check with your midwife or GP before starting an exercise program.

How Nursing Helps You Bond With Your Baby

Postnatal 'Confinement' Food - Part 2


Everybody will ask you to eat only ' healthy' food and avoid cold or cooling food as your body is cold after childbirth. This applies to the 3 main races in Malaysia but sometimes, what is considered 'healthy' may be 'cooling' for another which increases our dilemma. The best option is to try out the food as far as your culture and religion advices and learn to substitute the ingredients wherever possible to suit your taste. I am afraid that my culinary expertise is limited to Chinese and Western food so I can only talk about this type of food at this moment in time.

Traditional Chinese confinement food consists mainly of old ginger, rice wine, sesame oil, black vinegar, some warming alcohol, lots of meat especially kampong chicken and lean pork, white fish such as white pomfret and some vegetables.

Apparently, you are supposed to eat 30 chickens, 12 bottles of rice wine and 15 kilos of old ginger root during this period. Whether you can achieve this target is up to you but I find that most women nowadays cannot manage it. On average, if you do practice what is recommended, the cost could easily be about RM 1000.00 ++ just for the food only.

Food to avoid during the first week only

Ginger - Delays healing of wounds and increase risk of jaundice in baby. Substitute with peppercorn or garlic.

Fish - Causes foul smelling lochia or discharge.

Alcohols which are recommended for consumption during the confinement month are: Brandy, Yomeishu, rice wine and Guinness Malta because they are 'yang'. If you are a non-drinker, this sudden consumption may cause alcohol rash, redness in the face and heart palpitations, so do take it easy. Alcohol does go to the baby through breast milk, so if you want a good night's sleep, drink the alcohol after your dinner. The content of alcohol is reduced during cooking therefore you might consider adding it to your food during cooking instead of drinking it neat. Guinness Malta is non alcoholic and has high iron content therefore is suitable if you are anemic. Pulut rice is rich in iron therefore is used to make rice wine for postnatal nursing mothers. The rice is low in fat and free of cholesterol with high dietary fiber to reduce risk of heart attack and diabetes. It is better known as Shaoxing wine.

Drinks

It is believed that iced or cold drinks may cause severe shock to the system and further weaken the 'cold' body therefore it is strongly recommended that boiled warm teas are consumed throughout the day as they are supposed to help heat up the womb. For those who do not believe in these teas you may try drinking Chamomile tea which is beneficial for the baby as well, because it helps to reduce wind

Soups

Soup is a nutritious replacement for water. You can use any meat for stock. The meat supplies fat that is essential for extraction of fat soluble nutrients. Drinking soup about 1 hour before breastfeeding may help to increase milk flow. Chinese soups are usually clear and easier to drink compared to creamy western soups and it is less fattening.

Below are some food ingredients that you might want to add to your diet during the postnatal 'confinement' month.

Boxthorn fruit or Chinese wolf berry is best known to correct poor eyesight and eye strain. Used in conjunction with other herbs it is effective for improving energy and strength

Chinese angelica root or dong kwai is supposed to nourish the female organs, alleviate menstrual pain and prevent hemorrhage.

Chinese yam (wai san) used in conjunction with meat and other herbs aids digestion, regulate sugar level and control inflammation of the uterus. The dried variety must be soaked for at least 15 minutes before use to remove the powder

Solomon's seal (yok chuk) is used to alleviate many ailments especially those affecting the throat and lungs

Sesame oil, besides being rich in Vitamin E, Iron & Calcium, helps heal the womb and rid it of "dirty residual blood".

Chestnuts invigorate vital energy. Resolves fatigue depression and irritability. High in phosphorous, selenium and calcium

Black vinegar is made from glutinous rice and lowers ph of food. It leaches calcium from bones cooked with it, therefore it is used to prepare high calcium food especially for breast feeding mothers and it also relieves a windy abdomen.

Dill has antibacterial and antispasmodic properties. It is a warming herb which aids digestion, gets rid of gas, encourages lactation, freshens the breath and soothes colic. It is the main ingredient in Gripe water.

Almonds and walnuts contain leucine and isoleucine which are amino acids that regulates growth, blood sugar and wound healing.

Malay confinement food is the same as normal but with a lot of jamu which is basically natural herbal remedies used internally and externally for health and beauty. Make sure the products are registered with the Ministry of Health and that they are locally produced

Kani seeds contain antioxidants to promote youthfulness, tighten vaginal muscles, prevents excessive discharge, odour and itchiness. It firms the breasts and promotes healing of the reproductive system after childbirth.

Gallanggal is a member of the ginger family and it functions as an antibiotic. It warms the body, cleanses the blood, improves appetite, reduces wind and energizes the muscles.

Pegaga leaves increases energy, helps ease sleeping problems, improves blood circulation.

Turmeric functions as an antibiotic and promotes a healthy circulatory system.

Drumstick leaves are rich in proteins, vitamin C and A, beta-carotene, iron, calcium, potassium and other minerals. Because of their nutrients and ability to encourage milk flow, they are a valuable food for survival in impoverished countries.

The most important thing to remember is that you must be comfortable with what you eat. Do not force yourself to eat something that you do not like or will cause problems later. Although you are 'in confinement' you are not a prisoner per se.

High Blood Pressure and Pregnancy What You Should Know


A positive lifestyle change is a great way for the average person to control their blood pressure. However, what about pregnant women with high blood pressure? Can having high blood pressure during pregnancy harm the unborn child?

Yes. High blood pressure can result in complications during pregnancy that can be dangerous to both the mother and fetus if left untreated. There are different ways pregnant women with high blood pressure can help control their condition and reduce the health risks to the fetus. That being said, not all pregnant women with hypertension put their baby and themselves in danger. Many have healthy pregnancies without complications.

How common is hypertension in pregnancy? In the U.S. alone, it is estimated that problems with high blood pressure occur in 6 - 8% of pregnancy, over 65% of which are in women experiencing pregnancy for the first time.

What are the negative effects of hypertension? The effects high blood pressure can have on a pregnancy may be mild or severe and could include:

o Damage to the kidneys and other organs in the mother

o Cause early delivery and low birth weight

o In serious cases, the mother can develop preeclampsia (toxaemia of pregnancy), a dangerous condition that can be fatal for both the mother and fetus.

What is preeclampsia? This is a condition that usually develops in the 6th month of pregnancy and occurs from a combination of high blood pressure and kidney problems that cause protein in the urine of the mother. Preeclampsia affects the placenta and can affect the kidney, liver and even the brain of the mother.

When it affects the brain, preeclampsia can cause seizures, a dangerous condition called eclampsica which is the second leading cause of maternal death in America. Preeclampsia can also cause complications with the fetus including premature birth, low birth weight and stillbirth.

Preeclamspsia can be treated, but cannot be cured until the mother delivers the baby. There is no known way to prevent the condition, but those who show signs can be effectively treated to reduce risks.

Who is at the most risk for developing preeclampsia?

o Women with a pre-existing condition of high blood pressure prior to pregnancy

o Women who had preeclampsia or hypertension during another pregnancy

o Women who are obese

o Women who are carrying more than one child

o Women who become pregnant before the age of 20 and after the age of 40.

o Women with kidney disease, lupus, diabetes, scleroderma and rheumatoid arthritis.

Tips for controlling high blood pressure before becoming pregnant and while you are pregnant -

o Healthy Lifestyle - Make sure your blood pressure is checked regularly, you are an ideal weight, and live a healthy active lifestyle.

o Educate yourself - Talk to your doctor about your hypertension and find out what you can do to help prevent and reduce complications during pregnancy.

o Medications - If you are taking medication to control your blood pressure, find out if they are safe to take during pregnancy. Don't assume that they are safe, or will be dangerous and stop taking them without first consulting your doctor.

o Engage in regular medical care - when you are pregnant make sure you attend all regular checkups.

o Avoid alcohol and tobacco - this can increase blood pressure and harm the fetus.

o Discuss all medications with your doctor - Do not take any over-the-counter medications (even if you have a cold) without first consulting your doctor. Cold and flu medication often contains decongestants that can increase blood pressure.

Remember, although hypertension can cause complications in pregnancy, many pregnant women with high blood pressure, and even those who develop preeclampsia, often have healthy happy pregnancies and give birth to healthy, happy babies.

Blue Shield of California is a health insurance company that was established in 1938 by the California Medical Association. Based out of San Francisco California, the company services over 3.2 million members and is contracted with nearly 59,000 and 350 hospitals across the state.

With Blue Shield of California you have a couple different styles of coverage to choose from. More than likely you have heard the terms PPO and HMO thrown around. What do these terms mean?

PPO

A Preferred Provider Organization (PPO) gives you the availability to choose from a much larger network or doctors, specialists and hospitals. By visiting a contracted PPO provider with Blue Shield of California you will receive a negotiated fee rate for covered services. It is always important to make sure the doctor, specialist or hospital you choose is contracted with the company before you receive care. If you happen to pick an out-of-network provider then you may be subject to higher cost for service.

HMO

A Health Maintenance Organization (HMO) is a smaller organization of healthcare providers that have contracted with an insurance company to offer services at fixed rates. You will be required to choose one doctor (primary care physician) who will be responsible for providing all aspects of your healthcare. Blue Shield has 2 different HMO plan options available in California on the individual and family plans network. You can expect HMO doctors that are contracted with Blue Shield to offer fewer providers than with the PPO plans. Generally you must get a referral from your primary care physician in order to receive care from a specialist.

Let's take a look at some of the more popular plans that are currently being offered in 2009.

PPO Savings Plan 4000

This plan is great for a health young male who doesn't require many office visits per year. This plan doesn't include maternity, however we do recommend it to females who don't want maternity coverage or are past child bearing years. With most health plans you must satisfy a deductible and then pay a co-insurance percentage. With the PPO 4000 you are covered at 100% after meeting the calendar year deductible! You also have the availability of a physical exam, once a year for a co-payment of $35. If you are someone looking for affordable catastrophic coverage, this plan is highly recommended.

PPO Balance Plans 2500, 1700 and 1000

With the balance plans you have a good balance of deductible, office visit co-pays, prescription drugs, and a decent monthly premium. Each deductible option (2500, 1700 and 1000) comes with first dollar office visit co-pay of $30. There is also $10 co-pay for generic drugs and $35 co-pay for brand drugs ($500 brand name deductible and $2500 brand maximum benefit per year). If you happen to be the type of person that visits a doctor more frequently than once per year, and needs prescription drug coverage, take a look at the balance plans!

Access HMO

The Access HMO with Blue Shield of California is one of the better benefit plans on the market, however the monthly premium tends to be to unaffordable for most. After you satisfy a $2000 deductible you will have a $250 co-pay for impatient and outpatient hospital stays until you satisfy a total of $3000 in out-of-pocket expense (deductible included). Office visit co-pays are $20 before deductible; you will pay a $10 generic drug co-pay and a $35 brand name co-pay after a $200 brand name drug deductible. Remember, this HMO plan will have a smaller network of providers compared to the PPO options. Make sure to verify that all providers you use are a contracted physician or hospital with Blue Shield before receiving care.

This depression after childbirth is commonly known as postpartum or postnatal depression. This is a mixture of emotional, physical and behavior changes that affect the mother after delivering her baby. Depression can also occur during pregnancy, but it is more common after delivery of the baby.

A lot of things change after delivering the baby, the amount of blood in the body, immune system and the blood pressure all change. These changes can alter how a person feels about themselves, which can result in fatigue and mood swings. The exact cause of childbirth depression is not known, however as not every woman suffers from it, it's likely that lifestyle affects it.

This depression often goes undiagnosed and untreated, because doctors just don't notice these effects as being anything abnormal. Tiredness, sleeping problems, and changes in body weight are all normal changes, that happen to a woman's body during and after pregnancy, however these symptoms can also indicate depression.

Postnatal depression can be caused by:
Substance abuse.
Lack of support.
Previous pregnancy complications.
Young age of mother.
Doubting yourself.

The most common cause of depression after childbirth is drug or substance abuse, lack of support from family members, problems with previous pregnancies, mother's young age, and doubting your own ability to look after your baby. You can see most of the causes of depression are emotional and not physical.

Women need to snap out of the idea of having to be the perfect mum, because this can add to the stress. Also spending less time alone with your husband can cause depression after childbirth.

Baby blues and depression are two different things. Baby blues happen straight after childbirth, and normally disappear in under a week, but postnatal depression can strike any time during the first year and often doesn't go away.

Depression has no Rules

It is best to arm yourself with this knowledge prior to having the baby because it is difficult to recognize the signs of depression once you are living through it. The more you know about the issue prior to giving birth, the more likely you are to know when you need help and when you are okay.

The time to seek help for postpartum depression is the moment you start to suspect you may be suffering from it. Once you notice any symptoms or start feeling like you aren't bouncing back to your pre-pregnancy emotional state, it doesn't hurt to bring it up to friends, relatives, your spouse, or a doctor.

Those who love you should always be aware of how you are feeling so they can help you recover. Never try to hide feelings of sorrow or desperation!
